PROOF COPY APPROVAL/REJECTION FORM

All orders for which a proof copy was provided must be approved via this form for the order to move forward to the plant for production and shipping. If changes or corrections are needed please indicate "REJECTED" and then specify in the comments section at the bottom of form what changes are needed. YOU WILL THEN RECIEVE A REVISED PROOF, again requiring approval or rejection via this form. We can not proceed with your order until we are certain that we have understood your instructions correctly and that the layout we've provided is "OK to proceed AS IS"

All due diligence has been used in the preparation of your Proof Copy & Acknowledgment, but errors sometimes occur. The purpose of this form is to make certain that the details of order are accurate. Any failure on buyer's part to detect errors and advise us, do not relieve buyer of charges applicable to the order. In other words, if YOU miss an error and instruct us to "Proceed As-Is" you are still liable for the purchase amount, and will be billed accordingly. Thank you for your understanding.
